Clinico-pathologic Characteristics and Survival Rate of Malignant Ovarian Tumor

Abstract
The incidence of malignant ovarian tumor ranks fifth among malignancies of women and second among the gynecologic cancers. Malignant ovarian tumor is the most frequent cause of death on patients with gynecologic cancer because half of cases are
detected
in advanced stage. Late detection reflects an evidence of specific symptoms as well as the lack of effective strategies for prevention or screening.
Therefore we evaluated clinico-pathologic characteristics and survival rate of 140 malignant ovarian tumor treated at Department of Obstetrics and Gynecology of Kang-Nam Saint Mary's hospital during the period from May. 1980 to Jul. 1994.
@ES The results obtained were as follows.
@EN 1. In age distribution, 48.6% of patients were over age 50, including 30.0% for fifth decade.
2. In parity distribution, 20.4% of patients were in parity 4 or more, and 14.2% were in parity 0.
3. The common chief complaints were lower abdominal pain and palpable abdominal mass(43.8% & 35.%, respectively).
4. The bilateral involvement occured in 37.1%.
5. In size distribution 77% of patients were below 10cm in diameter.
6. The distribution of malignant ovarian tumor by stage was 39.0% for stage I, 13.0% for stage II, 26.8% for stage III and 21.1% for stage IV.
7. The distribution of malignant ovarian tumor by histologic type was 78.4% for epithelial tumor, 6.4% for germ cell tumor, 7.3% for gonadal stromal tumor & 1.6% for metastatic tumor. Epithelial tumor composed with serous type 60.2% and mucinous
type
32.7%.
8. Among the tumor markers, CA-125 revealed the highest positive rate(62.9%), the positive rate of CEA and a-FP were 21.4% and 8.4%, respectively.
9. The surgery was performed in 89.3% of cases, and among the operation procedures, total abdominal hysterectomy with bilateral salpingo-oophorectomy was most frequently used in 31.2%.
10. In the postoperative adjuvant treatments, chemotherapy was done in 91.5%.
11. Overall 5-year survival rates was 53%, According to clinical stage, 3-year survival rates for stage I was 79%, 70% for stage II, 50% for stage III and 20% for stage. IV. According to CA-125 level, 2 -year survival rate for lower level
(35¡ÂCA-125<300U/ml) was 70% and 64% for higher level(300U/ml¡Â).
